§ 49-506. Application for permit.
The owner of the news rack shall submit to the manager a signed application in writing upon a form provided and a certificate of insurance, approved in form and amount by risk management, showing the city as an additional insured covering any liability arising out of the maintenance of a news rack. The application shall contain:
(1)
The name, address and, if applicable, electronic mail address of the applicant;
(2)
The proposed location of each news rack for which a permit is sought. When the application is for more than ten (10) locations, the applicant shall supply a list on a computer disk formatted for use on the department's computer system;
(3)
Whether the applicant would prefer to receive notice and orders by registered mail, return receipt requested or will accept posting on an electronic bulletin board as notice; and
(4)
Shall be signed by the applicant.

Shall include the required permit fees.
